While some dentists restrict their practice to a particular age group, those with a family dentistry practice do not. Instead, they work with all ages to assist in the general maintenance of healthy gums, teeth, and overall oral hygiene. If you have a small child, however, you may be wondering if a general or pediatric dentist is best.

Family Dentistry

Many consider family dentistry the first line of defense against dental abnormalities. These practices serve as the starting point for checkups every six months, providing deep cleanings to eliminate buildup. In addition to visually inspecting your mouth, the doctor may also take X-rays to determine if there are cavities or other problems.

A person in this field must first earn an undergraduate degree, usually in an area of science, although this is not required, and pass the Dental Admission Test. After this, the student endures an additional three to five years of study at dental school. Prior to graduation, all students must pass the National Board Dental Examinations in order to obtain a license. Students graduate with a Doctor of Dental Surgery or Doctor of Dental Medicine.

General dentists can see children of all ages and even see adults. This is great for households that want to keep all their children going to the same person as they grow. It makes it easy for you as parents to also make time to see the doctor to care for your oral health.

Children do have special needs as they mature. Doctors trained in family dentistry know and understand these needs. They can treat loose teeth and cavities just as well as pediatric specialists. Additionally, they are able to prevent many dental problems with fluoride.

These doctors do not attach or manipulate braces. However, they can diagnose when your child needs braces and set you up with an appointment with an orthodontist.

Pediatric Dentist

If your child has special needs, your dental office may refer you to a pediatric dentist. These doctors have had additional post-graduate training.

If your child is very small or has a hard time at the doctor, then your dentist may suggest pediatric dentistry.

As with a general dentist, this professional cannot attach or maintain braces. They can help with other preventative measure and refer you to an orthodontist if necessary.

Family dentistry is a great place to start if you want one doctor to see all your children regardless of age. They can help your child maintain healthy teeth and gums and get them on the road to great oral hygiene.

What is a Capsular Contracture?

Capsular contracture is a rare, but concerning complication of breast augmentation. While breast augmentation continues to be one of the most common aesthetic procedures undertaken in the USA, capsular contractors can and do occur. Capsular contracture refers to a process where your body forms an abnormally hard shell around your implant, causing pain, discomfort, and loss of the breast shape. While this complication is obviously concerning, we routinely treat this unfortunate complication at our facility for all types of patients. As with all types of plastic surgery, treatment of this problem begins with defining the problem and how it specifically effects you.

4 Stages of Capsular Contracture

Grade I Capsular Contracture

To begin, all breast implants cause creation of a capsule. The magnificent machine that is the human body recognizes the presence of a foreign body internally. It then creates a soft capsule around the breast implant that is completely organic and physiologic. This capsule is actually quite helpful for both patient and plastic surgeon, as it creates a permanent “home” for the breast implant. This type of capsule is considered a Grade I capsule. No treatment or surveillance is necessary at this stage. This is actually what both patient and plastic surgeon want to have. Grade I breast is normally soft and appears natural in size and shape

Grade II Capsular Contracture

When the capsule begins to thicken up, and the implant feels slightly firmer, but does not change its overall shape, the capsule is then considered a Grade II capsule. The good news is that surgery is usually NOT indicated. In these instances, we treat the capsule through medical and mechanical means only. Lymphatic massage, ultrasound therapy, and the use of Zafirlukast can often reverse thickening at this stage, and salvage your augmentation. Grade II breast is a little firm, but appears normal.

Grade III Capsular Contracture

The next stage of capsular contracture involves continued hardening of the implant capsule, but now the hardening has changed the overall shape of your implant and/or breast. Grade III breast is firm and appears abnormal. This type of contracture is considered Grade III, and is obviously extremely distressing to women, but it can be treated!! Treatment involves a careful balance of medical and surgical management, and as always, it relies upon wide open communication between our staff and you. In this stage, the capsule has created a mechanical compression of the implant. These patients do require surgical revision. As always, the type of surgery used is individualized to the patient, and we have a number of different approaches to choose from:

  • Partial Capsulectomy – This procedure involves removal of only a portion of the capsule. This is our least common procedure for treatment of contracture, but it does play a role for some patients who have a focused area of scar contracture. Typically, this type of procedure is done on patients who have not had appropriate unilateral descent of their implant. Surgery involves removing only the offending area of capsule. Post-operatively, patients are started on lymphatic massage and medical therapy. In appropriately selected patients, this treatment is very effective, but not all patients are candidates for this approach. We do not use surgical drains with this procedure.
  • Total Capsulectomy – This operation removes the entire capsule and all associated scar tissue with your existing implant. Once the implant and capsule are removed, a new implant is placed after the entire area is irrigated with a triple antibiotic solution. This type of procedure may also require a breast lift be done simultaneously, depending on the shape and size of the breast.
  • Neopectoral Pocket – This operation uses the abnormal capsule to our advantage! In this procedure the capsule is meticulously dissected free from the surrounding breast tissue and muscle. Once this is complete, the capsule is then collapsed down to the chest wall and oversewn to provide more projection and volume to the new implant. This is a fantastic approach to contracture, and it provides some of our best results. Unfortunately, a small 8mm surgical drain is required on each side for this procedure. The drain usually removed between days 3-5 after surgery. This is a highly specialized procedure that we perform.
  • Implant Plant Exchange with Strattice – There is a great deal of long-term clinical data that suggests that implants placed above the muscle may have more capsular contracture than implants placed below the muscle. If you have an above the muscle implant with contracture, the treatment of choice is total capsulectomy, removal of your current implant, and placement of a new implant beneath the muscle with a Strattice sling. This procedure is our most common form of revision, as it provides a soft, natural look to your breast. Strattice is a type of biologic mesh that revascularizes once inside your body. It essentially becomes part of the breast structure, providing a permanent internal support bra for your implant in its new position.
  • Implant Removal Only – This is a very viable approach to treating contracture, and while most clients wish to save their augmentation, many do not want to keep their implant. Removal of the entire implant without capsulectomy can be done with local anesthesia only. In this approach, the capsule is left behind, but it will commonly soften up over time. In cases where it continues to be hardened, secondary capsulectomy can be used at a later date to reduce internal scar burden.

Grade IV Capsular Contracture

The last stage of capsular contracture, Grade IV, involves a permanent dull throbbing pain, and commonly, a sense that the breast feels cooler to the touch. As with Grade III, your treatment options are the same. Patients again are maintained post-operatively on maximal medical therapy (massage, Zafirlukast). Use of total capsulectomy with Strattice sling is very common in this group. Grade IV – the breast is hard, painful to the touch, and appears abnormal

In summary, there are multiple treatment options available for women who have experienced capsular contracture of their breast implants. Treatment options vary from non-surgical treatment options to implant removal. Capsular contracture does not necessarily require permanent removal of the implant.

Pruritus disappears quickly after treatment in most individuals. No new lesions develop, and all evidence of infestation ordinarily disappears within 7 to 14 days. Approximately 20% of patients, however, are apparent treatment failures. In about half of these patients new lesions continue to appear, and one must assume that either therapy has been inadequately carried out or that reinfection has occurred. Such patients and all of their contacts should be retreated. In the other half, no new lesions develop, but itching and scratching persist at the site of old lesions. These individuals, most of whom are genetically atopic, have developed an itch­scratch cycle and will continue to scratch indefinitely unless topical steroids and antihistamines are used to break up the cycle.

Scabies, if left untreated, persists for years. During this time there is a gradual resolution of old lesions and subsequent development of new lesions. This prolonged course accounts for the colloquial name of the disease” the 7-year itch.”

Pathogenesis

Scabies is due to an infestation with the human variety of the mite Sarcoptes scabiei. The female of the species burrows within the stratum corneum, depositing eggs that over a 3-week period mature into adult mites. The adult mite is just at the threshold of visibility and can sometimes be recognized as a tiny red or brown-red dot at the end of an intact burrow.

Transmission of the disease ordinarily depends on direct person-to-person contact. In a small percentage of cases, however, the contagion occurs through the use of shared clothing or bed linen. Infestation occurs in individuals at all ages and from all socioeconomic groups, but for unknown reasons it rarely develops in blacks. Once present, the disease spreads by scratching; mites and eggs are transferred from one location to another via fingernail contamination.

Early in the course of infestation there is little in the way of host inflammatory reaction, but after several weeks, allergic sensitization with accompanying pruritus and inflammation takes place. The role of immunologic response in the resolution of scabies has not been adequately studied, but the occurrence of epidemics at 20- to 30-year intervals suggests that the development and waning of herd immunity may be important from an epidemiologic standpoint.

Therapy

For many years, lindane (gamma-benzene), sold under the trade names Kwell and Scabene, has been the treatment of choice for scabetic infestations. It is quite effective and relatively cheap, but systemic absorption does occur and is associated with the potential problem of neurotoxicity. Expression of this potential problem only occurred when lindane was used for infants, but for this reason its use has been generally supplanted by 5% permethrin (Elimite). Crotamiton (Eurax) and precipitated sulfur have also been used historically, but neither is as effective as lindane or permethrin.

Both lindane and permethrin are used in a similar manner. Patients and all of their close contacts are instructed to bathe and then apply a thin layer of the medication over the entire body. This is left in place for 8 to 12 hours, at which time the patient bathes a second time. Clothing or bed linen used prior to or during therapy is washed with soap and water in a normal fashion. No special attention is needed for furniture and other inanimate objects. A single treatment carried out in this manner will result in clearing of 80% of patients. As mentioned above, an additional 10% will require retreatment because of problems with therapeutic compliance or reinfection. The remaining 10% will require the additional use of steroids and antihistamines in order to break up the itch-scratch cycle.

Everywhere you look, you are bound to find, almost always, something alive. But if most of the times you can easily make the distinction between plants and animals, what about bacteria? In which category do they go? We all know they are living organisms..but are they plants or animals?

In biology, living organisms are classified by relationships between species, and the science of classification is called Taxonomy. Modern taxonomic classifications are based on the similarities in the genetic material that define each organism.

Bacteria are organisms that have a single cell, and can be found practically everywhere, in any environment, not to mention your own body. In fact, you need bacteria in order to survive, as they are involved in all processes of your body. Moreover, the human body may hold ten times more bacterial cells than human cells. Bacteria are often associated with diseases, but in fact most of them perform functions essential for our survival, like digesting food for example.

Until the middle of the twentieth century, the accepted taxonomy (developed in the eighteen century by Carolus Linnacus, a Swedish botanist) divided the natural world into three categories: animal, vegetable and mineral In the mid-twentieth century, however, a new classification was developed, which divided living organisms into five kingdoms, based on the structure of their cells: animals, plants, bacteria and blue-green algae, and fungi.

However, as scientists learned more about microscopic organisms, a clear distinction between plants and animals seemed no longer possible. Moreover, as more is learned about DNA, an even smaller structure, some scientists have reached the conclusion that all classifications, not only plant/ animal, do not work at all.

And to answer the original question, are bacteria plants or animals, here ‘s a clarifying answer: bacteria are neither plants, nor animals. Every plant or animal cell has a nucleus, where it manufactures DNA, the genetic material. However, bacteria don ‘t have a nucleus, their DNA floats around inside the cell.

So no, bacteria are not plants, and are not animals. Bacteria are just bacteria. When you think of it, making a certain distinction between plants and animals does not seem hard at all. Especially if you think about regular ones, not carnivore plants or things like that. But in the end, it turns out that things are not quite so simple. You have a clear idea in your head about what plants are, you know what animals are, and then you have bacteria. Neither plants, nor animals.

Hang their artwork and have an art judging. Award a prize to each child for something unique pertaining to their art such as best use of color, best drawing of a tree and so on.

4) Kids love parades and what could be more fun than participating in their own parade. Have the kids form their own parade by providing musical instruments (drums, maracas, bells, etc.) and streamers. Have the kids march around the party area singing popular kids songs or the birthday song!

5) Kids at this age (actually any age) have lots of energy. Be sure to have enough activities planned to keep them busy.

5th Birthday

2) Since your child is in school now you can plan a celebration at school as well as a party at home.

4) Cupcake decorating is a fun activity for kids. Provide already cooked cupcakes along with the fixin’s such as frosting (a couple different flavors), sprinkles, candy and writing gel. Allow the kids to decorate the cupcakes and then they get to eat their creations!

5) Girls love to play grown up at this age so have a tea party for the girls. Either find a locale that has tea time or do it at your house. Tell the girls in the invitation to dress up and then serve tea and finger sandwiches with the crust cut off. Discuss the ritual behind drinking tea such as putting your pinkie up!

10th Birthday

2) This is an ideal age to celebrate your child’s birthday at an amusement park. Invite a few friends. The kids are going to be old enough or tall enough to get on many of the rides. And most of the kids probably have experienced an amusement park by now and know what to expect.

3) Plan a chocolate party. What kid doesn’t love chocolate? A chocolate party is simple to plan because everything is chocolate such as chocolate birthday cake, chocolate, favors, chocolate treats, chocolate fondue or even a chocolate fountain.

4) A great place to get favors for girls at this age is Claires or any similar accessory store. You can pick up tons of inexpensive accessories such as clips, barrettes and jewelry.

5) Plan a girly party for your daughter. A girly party would entail manicures and pedicures along with snacks and a fun girly movie such as the Princess Diaries. Have a couple of games or Barbie playtime and your daughter will love it!

Playing softball competitively or recreationally requires a set of basic skills which include catching, throwing, fielding, hitting and base running. A dedicated softball player must take time to practice the right techniques in the following skills in order to confidently and safely play the game. Even seasoned softball players must continue practicing and drilling themselves on these skills in order to improve their performance.

Catching

The right catching skills allow a player not only to catch the ball without getting hurt but also to position him to throw the ball precisely. The fact is that catching and throwing are two of the fundamental defensive skills in softball. The ability to catch and throw a ball is very important as this enables a player to throw out a baserunner who is attempting to advance to the next base.

Throwing

As previously mentioned, throwing is one of the two most important defensive skills a player must learn in softball. A quick and accurate throw can make it hard for the opposing team to advance their runners or make a score. Softball players should practice the right throwing technique in order to hit their targets quickly, precisely and consistently.

Fielding

Fielding is another essential defensive skill that a softball team must master. Good team fielding makes it difficult for the opposition to score runs. Players are drilled on the right way of fielding a ball that is hit on the ground as well as in the air. Fielding drills also involve knowing where to throw the ball. Many times fielders must rehearse particular plays so they will know where to throw the ball if it is hit to them.

Hitting

Hitting is probably the most challenging and enjoyable skill to learn in softball. Hitting a small ball with a rounded bat out to a particular area – in the split of a second – is a technique that softball players must continually practice and perfect. A good hit requires hand-eye coordination, quick reaction time, right technique and confidence.

Base Running

Excellent base running is not just about speed, it is also about smart running. Even more important than simply moving around the bases is the wisdom to evaluate the current game situation. A wily base runner takes into consideration the number of outs and the fielding placements of the opposition. Fast and smart base running puts great pressure on the defensive team.

In softball, where running and sliding are often done, a good pair of softball cleats and a trusty pair of supportive, moisture-wicking softball socks are essential items.

Dentures are the tooth replacements available for people who are not good candidates for either dental implants or fixed bridges. A denture is a set of artificial teeth attached to a plastic or metal plate which fits inside your mouth.

Often called dental plates, dental false teeth or simply falsies, dentures are usually fabricated from porcelain or acrylic teeth mounted onto pink acrylic base that resembles the gums.

Dentures or what you call dental prosthesis can look very natural depending on the quality of denture selected and the arrangement of the false teeth.

Dentures provide a multitude of benefits.

Natural-looking Teeth: The main advantage is a set of fully functioning teeth that will look and feel so much like you natural teeth, making you feel more confident in public.

Improved Confidence: People have a higher sense of self-esteem and confidence when missing teeth are replaced with dentures.

Improved Speech: By replacing areas that are missing teeth, patients can talk easily with more clear and natural voice.

Better chewing abilities: A properly fit denture can significantly restore your chewing efficiency with a better occlusion (bite). The false teeth allow you to eat properly and enjoy a more normal diet without being relied solely on purees and soft foods.

Types of Dentures

There are two main types of dentures: Complete and Partial.

Complete or full Dentures: The full denture is referred to as the full flesh-colored acrylic base fits over your gums and covers the palate (the roof of your mouth). Full dental plate is used when you are missing an entire set of natural teeth in the upper (maxillary) and lower (mandibular) arch.

Commonly referred to as standard dentures, full dentures can be further classified as "Conventional" and "Immediate."

The conventional type of denture is designed to be worn a month after teeth are extracted and the gum tissue has begun to heal. Unlike conventional dentures, the immediate denture can be worn straightaway but will require some adjustments to fit properly during the healing process.

Overdenture is another type of full denture. This kind of denture is a full denture which is placed in the mouth and covers a few remaining teeth, or dental implants.

Partial dentures: Also known as a bridge, a partial denture fills the gap left by a few or missing teeth. These false teeth are attached to a pink or gum-colored base, usually made from either metal or plastic (acrylic).

There are two types of partial denture: fixed and removable.

A removable partial denture, also known as a bridge, may use metal clasps that attach to remaining natural teeth. Patients can remove and reinsert a removable partial denture when required without professional help.

A fixed (permanent) bridge replace one or more teeth by permanently attaching crowns to natural teeth, tooth roots and / or dental implant abutments for support. Bonded permanently to tooth roots, dental implants, or natural teeth, fixed dental appliances are not removed by patients. Fixed partial dentures are more expensive than removable bridges but are more stable. If taken care of properly and fitted perfectly, the permanent type of dentures may last up to a decade, and in some cases longer.
